Registering with the surgery
To register with the surgery:
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Can I fill in a paper form?
Yes, but only if you cannot use the NHS online services.
You may collect a paper version of the form from our reception team during opening hours. You will then need to fill in the form and return it to reception for processing. We aim to complete paper registrations within 2 weeks.
Can I access my information online?
Yes, please bring 2 types of ID with you if you can’t use the NHS App and would also like to register for online access directly.

Online services
The best way to access your patient record is through the NHS App, it is a free, secure and comprehensive way to manage your GP NHS information, medications and appointments.
NHS England are continuing to develop and improve the App and online access, so we strongly recommend that you use the NHS App to save time and get the most from available NHS services.
If, however you don’t have a smartphone and are not able to use the NHS App, you can register for limited online services access directly with The Orchard Partnership.
You will need to present yourself at your GP surgery along with 2 pieces of ID:
- A photo ID (such as passport or drivers license)
- An official document with your address, such as a utility or council tax bill (it must have been issued within last 3 months)
The receptionist will check your photo ID is of a true likeness and that the address matches that which is on your record. You will then be issued with a username and password.
You can then use your login to manage repeat medications, manage appointments and update your contact details.
